My guess, based on what I've seen happen to countless undergrads... She turned on the power, set it for direct current, turned the knob for the B-volts and either didn't have the perfect buffer/medium for the current to flow through, or else she turned the volts up too high without paying attention to the Amps. Things can overheat in seconds. One of the most common scientific accidents in the world.
